Thank you.<\/p>\n<p data-start=\"0\" data-end=\"106\">Google Form Link: https:\/\/forms.gle\/BMACKe18WB1fgEzc6<\/p>\n<p data-start=\"299\" data-end=\"346\"><strong data-start=\"299\" data-end=\"336\">Our JPN Registration Office Code:<\/strong> 1401 7171<\/p>\n<p data-start=\"348\" data-end=\"368\"><strong data-start=\"348\" data-end=\"368\">Important Notes:<\/strong><\/p>\n<ul data-start=\"369\" data-end=\"719\">\n<li data-start=\"369\" data-end=\"457\">\n<p data-start=\"371\" data-end=\"457\">An email notification from our office will be sent within <strong data-start=\"429\" data-end=\"454\">14 days at the latest<\/strong>.<\/p>\n<\/li>\n<li data-start=\"458\" data-end=\"582\">\n<p data-start=\"460\" data-end=\"582\">If you receive an <strong data-start=\"478\" data-end=\"503\">automatic email reply<\/strong> after submitting the Google Form, this means your submission was successful.<\/p>\n<\/li>\n<li data-start=\"583\" data-end=\"678\">\n<p data-start=\"585\" data-end=\"678\">Please <strong data-start=\"592\" data-end=\"610\">wait patiently<\/strong> for our official confirmation email to arrange the specific time.<\/p>\n<\/li>\n<li data-start=\"679\" data-end=\"719\">\n<p data-start=\"681\" data-end=\"719\"><strong data-start=\"681\" data-end=\"719\">Do not submit the form repeatedly.<\/strong><\/p>\n<\/li>\n<\/ul>\n<p data-start=\"721\" data-end=\"744\"><strong data-start=\"721\" data-end=\"744\">Terms &amp; Conditions:<\/strong><\/p>\n<ol data-start=\"745\" data-end=\"978\">\n<li data-start=\"745\" data-end=\"883\">\n<p data-start=\"748\" data-end=\"883\">If the couple cancel the appointment and decides not to proceed the registration at KLSCAH, <strong data-start=\"844\" data-end=\"880\">the fees are non-refundable<\/strong>.<\/p>\n<\/li>\n<li data-start=\"884\" data-end=\"978\">\n<p data-start=\"887\" data-end=\"978\">If an <strong data-start=\"893\" data-end=\"908\">overpayment<\/strong> is made due to oversight, the <strong data-start=\"939\" data-end=\"977\">excess amount will not be refunded<\/strong>.<\/p>\n<\/li>\n<li data-start=\"884\" data-end=\"978\"><b>Only one representative<\/b> from the couple is required to fill out the form. If <strong>both parties submit separate forms<\/strong> and make duplicate payments, <b>any excess payment made will not be refunded.<\/b><\/li>\n<li data-start=\"884\" data-end=\"978\">For less popular wedding dates, a <strong>minimum of 2 couples is required<\/strong> in order for the date to be <strong>opened<\/strong>.
